Job Summary and Qualifications

What you will do in this role:

  • Responsible for providing lifelong support to patients following hematopoietic cellular transplantation
  • Serves as the primary point of contact for patients in the post-transplant setting
  • Coordinates all long term follow-up visits (LTFU), as specified by the program
  • Along with the acute allogeneic coordinator, ensures patients receives appropriate education and information prior to being discharged from the programAs appropriate, develops a survivorship care plan for all allogeneic patients at Day 100 and updates the care plan as health issues change

  • Contacts patients at routine intervals in post-transplant setting

  • Ensures patients remain on schedule with follow-up visits, vaccines and appropriate testing

  • Schedules post-transplant testing

  • Provides ongoing education throughout the complex delivery care process

What qualifications you will need:
  • Bone Marrow Transplant Certification BMT-CN, if not upon hire within 2 years of hire date.
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date
  • Registered Nurse
  • Registered Nurse Diploma, or Bachelors Degree, or Associate Degree

This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
