As a Palliative Care Coordinator, you will assume 24-hour responsibility for the coordination of care of patients throughout the hospital referred to the Palliative Care Team. The Palliative Care Coordinator will facilitate care of the patient in collaboration with the attending physicians, sub-specialists, nursing and medical teams, and ancillary services. He/She will establish an overall plan and will be responsible for coordinating all supportive care related services, educational and administrative, to ensure comprehensive care to all patients. The Coordinator will also be responsible for ensuring that the palliative care services for Methodist Healthcare System are in constant compliance with JCAHO. What qualifications you will need:

Provides hospital-based palliative care consultation for seriously ill patients and their family caregivers. All work is completed in close collaboration with physicians, nursing staff, and other team members.

Provides medical and psychosocial evaluation of patient and family; skilled and meticulous symptom management; extensive involvement in discharge planning and continuity of care efforts; and support and collaboration with primary medical staff responsible for the patient.

Provides in-service teaching of medical, nursing and other professional colleagues and students; role modeling for physicians and nurses at all levels of training; conducts weekly team meetings. Case finding initiatives will require strong interpersonal and collegial skills with colleagues from a broad range of professional disciplines throughout the hospital.

Demonstrates excellence in practice through full role functions – practitioner, educator, consultant, and leader.

Keeps abreast of physician utilization of service, their specialty and specific needs.

Participates in palliative care patient rounds to identify issues and evaluate solutions.

Actively participates in state and regional organizations that support supportive care program development.

What qualifications you will need:

Graduate of an accredited Associate’s or Baccalaureate Degree Nursing Program.

3 years RN experience in an acute care setting.

Current license to practice professional nursing in the State of Texas
