Job Summary and Qualifications

As a Clinical Pharmacy Specialist in Hematology/Oncology/BMT with up to one out of every three weekend rotation. This position has both inpatient and outpatient responsibilities, within a rotation. The position will be responsible for the development, implementation, monitoring and review of pharmacy services related to hematology/oncology. The focus of this position will be on direct patient care, developing best practices, safety and quality initiatives, therapeutic drug monitoring, protocol validation, order set maintenance, and participation in formulary management.

What you will do in this role: 

  • As a Clinical Pharmacy Specialist in Hematology/Oncology/BMT you will provide specialized pharmaceutical care through direct patient care in order to ensure the appropriate selection, dosing, administration and dispensing of medication orders
  • You will review and assess medication orders to identify, prevent and resolve medication issues and provide therapeutic drug monitoring
  • You will consult, educate and work cooperatively with internal staff to provide both formal and informal pharmacotherapy education that includes drug information, therapeutic evaluations, and medication protocols
  • You will monitor and manage adverse effects related to chemotherapy and biologic therapy
  • You will provide counseling to patients and family members on drug therapy and supportive care
  • You will provide clinical support for multidisciplinary committees, including the Pharmacy and Therapeutics Committee, through the development of materials to support clinical evaluation of drugs, procedures and processes
  • You will participate in treatment plan development and validation
  • You will identify and implement quality improvement initiatives
  • You will oversee transitions of pharmacy care between pediatric inpatient and outpatient settings
  • You will assist the Clinical Pharmacy Manager in maintaining compliance with Joint Commission and other regulatory standards
  • You will actively participate in pharmacy internship and residency training programs

    What qualifications you will need:

    • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy
    • Minimum of 3 years’ experience as Advanced Clinical Pharmacist (in a specific area of focus) or at least 5 years of general hospital pharmacy experience required; preferably within area of focus
    • Or completion of a PGY2 pharmacy residency or fellowship
    • AND BCPS certification or other Board of Pharmacy Specialty (BPS) certification
    • ONE of the following is required prior to the first day of employment: Current Registered Pharmacist licensed by the Texas State Board of Pharmacy or Licensed pharmacist in another state: Provide proof of the Authorization to Test (ATT) from the Texas State Board of Pharmacy.
    • ACLS/PALS and NALS required for critical care/ED position dependent on population served.

      This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
