Job Summary and Qualifications
As a Pharmacy Technician Medication History Specialist, you will be performing medication histories for new hospital admissions. Your duties are performed under general supervision of the clinical pharmacist, provide complete and accurate information for each medication, e.g., drug name, dose, route, frequency, indication, and date/time last dose was taken. You will utilize available resources for medication history collection including but not limited to: patient/family interview, medication lists, outside facility medication administration records, outpatient pharmacy records, and physician office records. You will verify/update patient allergy, adverse drug reaction, preferred pharmacy and primary care physician information in the electronic medical record. You will document medication histories completed in the electronic health record, sending to the pharmacist for cosignatory in an accurate and timely manner. You will document medication histories in CPW/VigiLanz in an accurate and timely manner. You will assist the medication reconciliation pharmacist by ensuring patients identified for discharge counseling and medication reconciliation have a pharmacy services completed medication history. You will prepare education materials for the medication reconciliation pharmacist as necessary. Your schedule may vary according to the needs of the facility/department and may include weekend, evening shift, holiday, on-call, and additional hours.
What you will do in this role:
- As a Pharmacy Technician Medication History Specialist, you will perform medication histories.
- You will interview patients and family members for the purpose of compiling an accurate and complete list of current medications the patient was taking prior to admission to the hospital, to include all prescription, non-prescription and investigational medications.
- You will contact the patient’s community pharmacy, physician’s office, or other healthcare facility for clarifications if needed.
- You will accurately update medication history, allergies, adverse drug reaction, pharmacy and primary care provider information in the electronic health record and CPW/VigiLanz.
- You will correctly and completely document interventions made to the medication history in the electronic medical record prior to the end of each shift worked.
- You will discuss any questions or issues that arise during the medication history process with a clinical pharmacy specialist.
- You will appropriately communicate unresolved medication history issues to clinical pharmacy specialists.
- You will ascertain need for language assistance and obtain interpreters as necessary.
- You will assist the clinical pharmacist performing education and/or discharge counseling through identification of patients that need medication histories completed by pharmacy services and preparation of counseling materials.
- You will attend and participate in staff meeting regularly (full-time and PRN staff). You will obtain prior approval when unable to attend.
What qualifications you will need:
- High School graduate or GED
- 2+ years previous retail pharmacy experience
- 1 year previous inpatient pharmacy experience preferred
- Maintains current pharmacy technician registration or pharmacy intern license.
- Certification as a Pharmacy Technician for registered pharmacy technicians; a 90 day grace period is allowed after hiring into the role
