Job Summary and Qualifications

LVN FT Nights Cardiovascular Telemetry Services 7p-7a Rotating Weekends

The Licensed Practical Nurse/Licensed Vocational Nurse (LVN/LPN) delivers high quality, patient-centered care by performing a variety of delegated basic patient care services. In collaboration with the RN and other members of the care team, the LPN/LVN provides individualized, comprehensive care consistent with the scope and standards of the specialty.

    Scope for Cardiovascular Telemetry Services:

    Primary admitting diagnosis is “cardiac”: New onset CHF, CHF exacerbation, AFIB RVR, New Onset arrhythmias, Chest pain, Angina, PPM placement, PPM firing, PCI, STEMI, NSTEMI, Hemodynamically stable pericardial effusions. Goal of unit is to have all Heart Failure, and PCI patients targeted to CVTS to provide consistent patient care and improve outcomes with consistent process implementation.

    Drips: (All cardiac drips must be ordered at set rate):Amiodarone protocol, Bumex, Lasix, Heparin, Cardizem, Angiomax, Nitroglycerin, Cardene, Dobutamine. Patients must not have labile blood pressures while on cardiac drips. Should patient require titration of drips, level of care will be escalated appropriately.

    CVTS Competencies: Chest tubes, Post PCI recovery, TR-Bands, heart failure, AF RVR, permanent PPM management, NIHSS for CVA

    In addition, we will be recovering transradial cath patients

    Assessments and frequencies:

    ·Vitals q4

    ·NIH q4

    ·Neurovascular q2 when hemodynamically stable with goal to q4

    ·Vascular 4h

    ·Accucheck q2 when stable and not requiring endotool

    ·EKGs per Tikosyn protocol and New onset Chest Pain admissions

    ·PRN Bipap without continuous SPO2 monitoring (subject to change pending equipment and patient respiratory status)

    ·Continuous Bipap with parameters for successful weaning to PRN within 24 hours of initiation.

    ·High flow Oxymizer when stable

    Staffing grid

    Census/Volume: 33 beds, all private rooms.

    RNs: 8 (1 free charge, 7 floor nurses with ratio of 4-5 patients)

    PCTs: 3 per shift

    Unit Clerk: 1 ( swing shift)Restorative tech: 1 (primary function is to ambulate patients


    What you will do in this role:

     

    • Provide direct, evidence-based care to patients as directed by the physician or RN.
    • Assist with procedures, monitoring, or other functions as directed.
    • Perform lab and procedural tests within the LVN/LPN scope of practice, after demonstrating competency.
    • Document the administration of care in the patient medical record in a timely and thorough manner.
    • Serve as a resource to department staff, students and volunteers to help deliver quality patient care.


    What qualifications you will need:


    • Basic Cardiac Life Support must be obtained within 30 days of employment start date
    • Licensed Practical / Vocational Nurse (LPN / LVN) Texas State or Compact State license required
    • Vocational School Graduate
    • 1-2 Cardiovascular Telemetry / Cardiac experiences acute care hospital nursing experiences.
