Job Summary and Qualifications
Primary responsibilities include coordination and follow-up of patient care for assigned orthopedic patients. This includes serving as a resource to inpatients, outpatients, nursing staff, and physicians while assisting with implementation of growth strategies and process improvements to maintain accreditation by regulatory agencies. Other responsibilities include coordination and optimization of hospital-based care and facilitation of post-discharge and continuity for orthopedic patients. Working with hospital case management and other members of the multidisciplinary care team, the Orthopedic Navigator facilitates assessment, care plans and discharge planning. Assists the patient, family, case managers and physician in procuring medical and supportive resources, and provides a point of contact for the patient and family for information and problem solving. Maintains strong relationships with orthopedic surgeons, hospitalists, general surgeons, cardiologists and other specialists, and facilitate coordination and problem solving for any patient issue. Maintains an accessible services tracking system and generates updated weekly and monthly reports for hospital administration. Demonstrates team work and adaptability through consistentlyfollowing the standards of Methodist Excellence.
- Advanced Cardiac Life Support must be obtained within 60 days of employment start date

- Associate Degree, or Bachelors Degree, or Registered Nurse Diploma
