Your Role:  Registered Nurse (RN) Clinical Nurse Leader (CNL)


The RN Clinical Nurse Leader (CNL) is an advanced generalist prepared at the Master’s degree level. The Clinical Nurse Leader role involves the holistic knowing of patients, enhanced communication amongst caregivers and lateral integration of care. The Clinical Nurse Leader possesses a specialized knowledge of horizontal leadership and is accountable for the management of care and the care environment. The Clinical Nurse Leader provides leadership in the application of the nursing process to client care and organization processes and/or systems. The Clinical Nurse Leader is also responsible for a mix of staff ranging from RNs, LVNs, and PCAs. The Clinical Nurse Leader shall establish a strong partnership with the mix of staff to deliver optimal care ensuring safety and quality outcomes with 24-hour accountability for the care provided by self and others to who care is delegated. Plans for and obtains the equipment and supplies needed for assigned procedures according to physicians' preference and specific patient needs.


Clinical Care:
  • Provides centralized care coordination of caseload of patients at the micro system level
  • Oversees the management of patient care through assessment of patient population daily
  • Rounds with clinicians daily (nurse and physician) to discuss patients progress
  • Facilities direct care when appropriate to complex patients
  • Conducts interdisciplinary team meetings to provide mechanism for all clinical disciplines to collaborate, plan, implement and assess the plan of care; patient selection should be criteria-based and interventions will be documented
  • Troubleshoots complex problems in patient care, documents findings and progress in patient record
  • Ensures patient/family preparation for care across continuum

Outcomes Manager:

  • Reviews consultation notes and/or speaks directly with consultants to discuss care
  • Assesses patient’s response to treatment and care through other care provider information or examines patient directly
  • Consults with physicians and other healthcare providers to communicate patient progress
  • Collects data on patient outcomes on the unit i.e. falls, pressure ulcers, medication errors, etc.
  • Member/Chairs Unit Based Quality Committee      
  • Disseminates and implements latest evidence-based practice to nursing staff and colleagues
  • Writes or revises unit-based procedures, protocols to reflect latest evidence
  • Serves as a liaison with facility-based quality committees. Works with nursing staff and physicians to ensure compliance with core performance standards
  • Analyzes cost effectiveness of planned care
  • Implements research findings determining costs and client benefits i.e. Evidence-based practice
  • Assumes a leadership role in the development, revision and implementation of clinical protocols which transition patients across the continuum of care

Patient Advocate:

  • Ensures that patient goals are set with patient and family/significant other’s involvement

  • Negotiates healthcare resources on behalf of the patient
  • Educates patient/family regarding available options for care through the continuum
  • Educates patients and family about Advanced Directives
  • Supports patient’s right to make decisions about care and treatment
  • Facilitates full team discussions including patient and family when ethical dilemmas arise


  • Utilizes the most current tools to access information for teaching
  • Ensures appropriate assessment of learner needs
  • Creates and coordinates the development of patient educational pathways and patient education materials
  • Provides input as committee member or clinician to patient education material development
  • Consciously provides opportunities for less experienced nursing staff to observe and participate in physical assessment, data review and evaluation of individual outcomes provided by the Clinical Nurse Leader
  • Engages the staff in clinical rounds
  • Provides formal and informal educational opportunities on the unit to improve clinical skills of all stuff

Information Manager/Flow of Communication:

  • Utilizes information technology to obtain evidence-based practices and the latest research to provide current best practice information to care providers on the unit
  • Develops and utilizes tools available to effectively evaluate, summarize and communicate patient progress towards outcomes to members of the healthcare team and patient and family
  • Communicates between caregivers in a timely, continuous and complete manner
  • Communicates constructive feedback to assist others in their growth as professionals
  • Is cognizant of cultural differences when communicating
  • Is able to change methods of communication as appropriate to patient and families ability to understand
  • Demonstrates an awareness of patient’s right to privacy when communicating with others
  • Maintains communication with Case Manager and Physician to ensure appropriate utilization of resources

Microsystems Analyst/Risk Anticipator:

  • Works with team members, other leaders and physicians to develop and trend performance improvement indicators based on national standards and best practices
  • Provides updates to Chief Medical Officer, VP of Quality and Chief Nurse Executive on clinical outcomes and action plans at least monthly.
  • Collaborates with the Chief Nurse Executive in preparing nursing clinical outcomes report to Medical Board and Community Board quarterly
  • Reports results on quality report card as well as storyboard and other forums
  • Conducts unit-based performance improvement initiatives to provide a safe environment for patients
  • Provides information to staff on nationally recommended safety practices
  • Assists Director with monitoring of the unit’s compliance with regulatory agencies requirements relative to patient safety including TJC, OSHA, CMS and others
  • Is involved in safety initiative at the department and interdepartmental levels to improve care outcomes within the micro system of care

Team Manager:

  • Provides input to nursing leadership on the evaluation and competency of nursing staff
  • Recommends specific programming to build clinical skill set of staff
  • Coordinates the delivery of care of the multiple disciplines actively involved in the patient’s care
  • Ensures the effective education of the patient and patient’s family to prepare for discharge
  • Endures effective communication through the continuum to support ongoing progress toward identified outcomes



  • Graduate of an accredited Clinical Nurse Leader (CNL) Master’s or post- master’s Nursing degree program


  • Five years of clinical acute care nursing experience, acute care nurse leader experience and/or acute care nurse educator experience.

Employee completes initial and ongoing training and competencies as defined by service line, facility and department/unit which are specific to the needs of the patient population served (if applicable)



  • Nursing Leadership and Nursing Educator experience


ALL of the following:

1. RN License to practice as a professional registered nursing in the state of Texas

2. Clinical Nurse Leader Certification from the Commission on Nurse Certification (CNC) of the American Association of Colleges of Nursing (AACN) within nine (9) months of hire date.
