Your Role: Behavioral Medicine Program Therapist II


The Behavioral Medicine Program Therapist II is a member of an interdisciplinary treatment team that provides direct age-appropriate clinical care and case management services to behavioral health patients. In coordination with other team members develops the patient’s individualized plan of care. Is responsible for conducting individual, group, and family therapy as needed, as well as completion of assessments and other required clinical documentation.

  • Provides age-appropriate individual, group and family therapy as well as psychoeducational groups as clinically indicated.
  • Provides case management services to patients to include discharge planning and linkage with community resources.
  • Completes integrated biopsychosocial and other assessments, as needed, in a timely manner.
  • Is responsible for the organization and completion of treatment planning reviews.
  • Communicates with team members, referral sources and community resources to coordinate services for the patient.
  • Completes required documentation in patients’ records in a thorough and timely manner.

 


Qualifications

Education: 

  •      Master’s Degree in Social Work, Psychology, Counseling or related field.

Experience: 

  •      Five years of experience in a mental health setting is required.

Employee completes initial and ongoing training and competencies as defined by service line, facility and department/unit which are specific to the needs of the patient population served (if applicable)

 

License/Certification:

  •      Licensed in the State of Texas as a Licensed Master of Social Work, Licensed Clinical Social Worker or
  •      Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT).
