Seeking a Performance Improvement Nurse - Core Measures to join our team who will performs concurrent review and retrospective in-depth analysis of patient records for purposes of core measure performance.  You will also interact with physicians, nursing, and ancillary staff to promote accurate documentation.

 

What you will do in this role:

  • Maintains an accurate knowledge base of the Core Measures and the outcomes to be achieved.
  • Performs detailed, concentrated work with limited supervision while accurately reviewing a patient’s hard chart as well as Meditech chart for Core Measures outcomes and/or delinquencies; must also be able to work as part of a team.
  • Communicates effectively verbally and in writing with physicians, nursing staff, and all other customers involved in Core Measures; takes action on individual cases not meeting core measures indicators.
  • Organizes, prioritizes and manages time efficiently to meet deadlines.
  • Uses problem-solving skills; coordinates with Quality Manager, and other areas involved with Core Measures to develop tools or to identify process improvement initiatives for improving outcomes.
  • Serves as a resource for nursing unit personnel, physicians, and other ancillary departments related to Core Measures.

 

 

 


 

 

 

Qualifications

Education:  

  • Graduate of nursing program:  Diploma, Associate degree, or Baccalaureate program.

Experience: 

  • At least 2 years experience in adult cardiovascular critical care/intermediate care or adult medical surgical area working an average of 24 hours/week.

License and Certifications

  • Current licensure to practice professional nursing in the state of Texas.
