Seeking a Patient Care Liaison who is responsible for the programs and resources that support the coordination of ED patients through complex healthcare environments for the optimization of care, outcomes, and patient experience. Responsible for facilitating patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. The Patient Care Liaison guides patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes while consistently providing exceptional customer service. Serves as a point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services.
What you will do in this role:
1.Reviews patient case lists generated by technology-enabled data triggers to identify patients that could benefit from intervention
2.Communicates with physicians (in the hospital and in the community) about patients' potential for referral / follow-up treatment
3.Provides feedback to leadership through daily communication and weekly status updates in regards to target population in order to enhance care coordination and patient experience.
4.Collaborates with care team members and suggests ideas for development.
5.Significantly improves specific processes; recommends and facilitates initiatives to improve stakeholder value.
6.Drives key business improvements and results.
7.Participates as a subject matter expert.
8.Demonstrates competency and compassion and performs consistently under routine conditions/situations.
9.Applies appropriate tools and methods to guide patients through the system.
10.Schedules follow-up appointments and provide information and reminders to patients & physician offices
11.Tracks outcomes of referrals and treatments
12.Maintains detailed records to measure effectiveness of program and suggests ways to improve throughout the process
13.Engages providers and patients in a program that will improve outcomes and the efficiency of medical care
14.Reviews clinical progress of patients and coordinates communications to referring, consulting, and primary care physicians.
15.Plan, organize and provide direct care to the patient, as appropriate and within scope, if called upon to do so.
16.Facilitates ongoing performance improvement activities related to the target population and involving all levels of staff
17.Updates patient and families regarding planned procedures, medications and treatments as needed.
18.Schedule appropriate referrals as noted by the physician.
19.Coordinates information among referring, primary care and consulting physicians and ancillary departments.
20.Answers patient/family phone calls, fields questions, and provides resources with empathy and compassion
21.Participates in creating an environment and performs job functions in a manner that exemplifies the mission, vision and values of the organization.
22.Encourages collegiality and collaboration when integrating pilot services with other departments.
23.Proactively assists patients with registration process as needed, offers comfort items as appropriate, and demonstrates anticipatory service to patients and families in the waiting area.
24.Evaluates and provides input regarding the utilization of resources to meet the needs of the target population.
What qualifications you will need:
Minimum Education, Experience, Training, Required Knowledge, Skill, Abilities and Other Job-Related Characteristics:
1.Education: High School Diploma
·Minimum 2 years of customer service experience or patient experience
3.Employee completes initial and ongoing training and competencies as defined by service
line, facility and department/unit which are specific to the needs of the patient population
served (if applicable).
